咖啡与自身免疫性肝病的“三生三世”!

     咖啡与自身免疫性肝病有什么联系呢?

     咖啡是世界上消费量最大的饮品,近几十年,由于咖啡对健康所产生的影响,这方面已经进行了大量的研究。最近的研究表明,咖啡可减少心血管病、神经系统疾病、2型糖尿病以及子宫内膜癌和肝癌的死亡率。通过查阅文献我今天来谈谈:关于咖啡与自身免疫性肝病的“三生三世”。

     自身免疫性肝病主要包括三种疾病:即自身免疫性肝炎,原发性硬化性胆管炎(PSC),原发性胆汁性胆管炎(PBC)。

     这些疾病有各自不同的靶组织,三者进展为肝硬化均不可避免。迄今为止,它们的发病机制仍不清楚。基因和环境的相互作用似乎增加了罹患这些疾病的易感性。环境因素,例如吸烟、感染、化学毒素和饮食摄入,与这些疾病的发病和病情活动有关。

     关于肝脏疾病,咖啡与延缓肝硬化有关。同样的,咖啡可以降低发生肝细胞癌的风险,以及降低肝硬化患者的死亡率。

     在自身免疫性肝病的对照性研究中,分别有24%的原发性硬化性胆管炎患者和16%的健康对照者不饮用咖啡(P <0.05)。在喝咖啡人群中,原发性硬化性胆管炎患者饮用咖啡少于健康对照组(45%对47%,P<0.05)。此外,原发性硬化性胆管炎患者生活中平均饮用咖啡时间(从一开始喝咖啡到停止)比健康成人相比较有所减少(46.6%比66.7%,P<0.05)。综上,这些结果强调了咖啡对PSC发病的保护作用。

     此外,患者每月饮用咖啡多于20杯与原发性硬化性胆管炎发病呈显著负相关(OR 0.89;95% CI 0.83–0.94,P<0.001)。

     另一项研究显示250名原发性硬化性胆管炎患者与对照组相比,原发性硬化性胆管炎患者喝更少的咖啡(76%比86%,OR 0.52;95% CI 0.32 - 0.82,P = 0.006)。此外,喝咖啡被发现与原发性硬化性胆管炎发病呈负相关(P = 0.048)。

     相比之下,饮用咖啡与原发性胆汁性胆管炎发病之间没有显著相关性。此外,溃疡性结肠炎患者合并原发性硬化性胆管炎的患者,喝咖啡可以降低发生结直肠切除的风险(HR 0.34,P<0.001)。

     结论:目前的研究表明咖啡对PSC发病有保护作用,无证据表明饮用咖啡与PBC发病有相关性。
